HopeWest is proud to introduce Ann Cromika, Hospice and Palliative Care Nurse Practitioner, who is now serving Montrose and Delta patients. With more than 20 years of healthcare experience, Cromika brings her expertise to our hospice and palliative care team. This team is made up of skilled professionals, each with unique roles designed to address the medical, emotional, and spiritual needs of patients facing serious illness or nearing the end of life.

“Ann’s compassionate approach to care gives her valuable insights into our patient’s care experiences,” said Kelly Thompson, HopeWest Program Director in Montrose.

Jenni Neil, HopeWest Program Director in Delta, compliments Ann’s vested interest in the patients she serves, saying, “She truly cares for their wellbeing and will do what it takes to make sure they are confident and comfortable with their care plan.”

Starting her healthcare career as a paramedic, and her experience in acute care nursing has equipped Cromika with a comprehensive understanding of patient needs. As a nurse practitioner, her primary responsibilities include assessing patients’ ongoing eligibility and developing plans to manage their symptoms. Her in-person evaluations provide valuable insights into staying apprised of patients’ ever-changing medical needs and ensuring the care they require is provided in a timely manner.

Ann Cromika is a board-certified Family Nurse Practitioner. She began her nursing career in 2007 and furthered her education to become an Acute Care Nurse Practitioner in 2012, earning her master’s degree from Colorado State University. She looks forward to using her experience to help patients and families navigate chronic conditions and gain quality time together.
